Advisory - RTH Validation Remark “WITH ERRORS ENCOUNTERED IN CF4 XML…” (CF4 XML Concern)

Dear Valued Client,

We would like to provide an update regarding the reported RTH validation remark: 
“WITH ERRORS ENCOUNTERED IN CF4 XML FOR REFERRAL TO SERVICE PROVIDER TO FIX AND RE-FILE”

Please note that this is a general remark typically issued when the concern is related to the CF4 XML file. On its own, this remark does not identify the exact root cause unless it is accompanied by an official Validation Report indicating the specific error details.

Upon verification, we recommend that facilities request the Validation Report/Error Details for the affected RTH claim from your designated PhilHealth Regional Office (PRO) and check whether the error description matches any of the known key phrases (sample shown in the attached Validation Report screenshot).

Current Guidance from Some PROs
While the evaluation and review are ongoing, several PROs have advised facilities to proceed with the following:
  • Request the Validation Report/Error Details from your assigned PRO for confirmation of the CF4 XML issue.
  • If the error description matches any of the known key phrases, please re-upload the CF4 XML and attach a PDF/A copy of the CF4 with the attending physician’s signature under the document type OTH or CAB.
  • Before regenerating and re-uploading the CF4 XML, ensure that all required CF4 details are complete, accurate, consistent, and correct.
  • Always double-check that the CF4 XML was successfully generated prior to submission.
  • Ensure that the return claim is successfully refiled after completing the required attachments.
Additionally, you may coordinate with your assigned PRO to confirm whether you are allowed to attach a PDF/A copy of CF4 during initial submission as a backup document. This may help reduce future filing delays in case CF4 XML-related issues recur.
